Which baptism certificate template do I need?
- Certificate of Baptism: for anyone, of any age, in any church. It reads that the person “was baptized in the name of the Father, and of the Son, and of the Holy Ghost,” the words of Matthew 28:19.
- Certificate of Water Baptism: for a believer’s baptism, when someone is baptized after confessing faith in the Lord Jesus Christ.
- Child’s Certificate of Baptism: for a child, with lines for the parents, the date of birth and the godparents or sponsors.
- Certificate of Dedication: for a baby dedication, in churches that baptize later. It says the child was dedicated to the Lord, not baptized.
The wording works across churches: none of the certificates names a denomination or says how the baptism was done.

“Buried with him in baptism, wherein also ye are risen with him through the faith of the operation of God, who hath raised him from the dead.”
How to print and fill in a baptism certificate
- Paper: white or ivory cardstock, 65 to 110 lb (176 to 300 gsm).
- Print at actual size (100%). The certificates are pages 3 to 6, in landscape; everything sits inside a half-inch margin, so home printers won’t clip it.
- Practice first: print a blank copy on plain paper and fill it in, so the names fit the lines.
- Fill in by hand in dark ink, and sign on the day: the pastor or officiant and a witness or parent.
- Seal: the dashed circle is for a church stamp, an embossed seal or a gold sticker.
- Frame it in a standard 8.5 × 11 in (or A4) frame.
A keepsake, not an official record. These certificates are for remembering the day. For proof of baptism, ask the church where you were baptized for a copy from its register.
A child’s baptism or a baby dedication
The child’s certificate has lines for the parents, the date of birth and the godparents or sponsors. Churches that baptize believers later can use the Certificate of Dedication instead. Like Hannah, who brought Samuel to the house of the Lord, parents promise, with God’s help, to raise the child to know and love Him.

“For the promise is unto you, and to your children, and to all that are afar off, even as many as the Lord our God shall call.”

“For this child I prayed; and the LORD hath given me my petition which I asked of him: Therefore also I have lent him to the LORD; as long as he liveth he shall be lent to the LORD. And he worshipped the LORD there.”
